Vaccine Information for 16, 17 & 18-Year-Olds
The Erie County Department of Health has provided this information on vaccination sites and parent consent for 16- and 17-year olds for school districts to share with their school communities:
While in general it is strongly encouraged that a parent or legal guardian accompany a minor age 16 or 17 years to provide in-person consent for vaccination at each dose, the Erie County Department of Healh has provided this consent form for parents who will not be attending a ECDOH COVID-19 vaccine clinic with their 16- or 17-year-old child, and who otherwise will not be available to provide consent by phone:
