District forms for use in fundraising activities are available below. Organizations wishing to host a fundraising activity are required to contact Christine Starks at (716) 926-2210 or via e-mail at christine.starks@lscsd.org for approval prior to beginning the activity. Please note that all fundraising activities require the submission of a post fundraising report form after the activity is completed.
