Physical Information To Participate In Sports
Athletes can go to their own physician for their physical or they can obtain a physical at the school. Each athlete, must obtain a form from the school nurse to take to their primary care physician to be filled out. This form must be used by our athlete and then be returned to either the nurse or the athletic office. The form must be turned in prior to the first day of try-outs. If it is not, then the athlete will not be able to participate until the completed form is returned to us as well as the Parent Permission form. The district will still provide athletes an opportunity to see the school physician if they cannot see their own primary care physician. The physical dates will be added as soon as we have confirmation from the doctors office.
The athlete only needs to have one physical per school year. If they participate in a sport during any other of our three seasons they only need to bring in an update form.
