Below is the link for the required parent permission form/health update that needs to be completed and turned in prior to athletic participation for every season. 

Updated Health Update/Parent Permission forms can not be turned in more than 30 days before the start of the season. We will begin collecting these after August 1st for the upcoming fall season.
