The Model Transition Program Job Readiness Curriculum
 

The Model Transition Program (MTP) Job Readiness Curriculum is designed to help students with disabilities become knowledgeable about the world of work, and includes modules on setting goals, the job hunt, first impressions, professionalism at work, managing work and personal life, confidence on the job, the new economy, and middle school preparation. The curriculum contains background information for the instructor, activities, handouts, and supplemental resources.

JobTIPS on do2learn site

JobTIPS is a free program designed to help individuals with communication, socialization, and learning differences explore career interests, seek and obtain employment, and successfully maintain employment. Individuals with an autism spectrum disorder (ASD) may find this site particularly helpful, as would individuals with learning differences, ADD, speech or language disorders, and other areas of exceptionality. JobTIPS addresses the social and communication differences that might make identifying, obtaining, and keeping a job more difficult.

Healthy Transitions NY

This website is for youth with developmental disabilities ages 14-25 years, family caregivers, service coordinators, and health care providers.

HealthyTransitionsNY.org teaches skills and provides tools for care coordination, keeping a health summary, and setting priorities during the transition process. It features video vignettes that demonstrate health transition skills and interactive tools that foster self determination and collaboration. 

Use this site to develop skills for transitioning from pediatric to adult healthcare.Our curriculum and tools raise awareness about developmental disabilities, improve communication, and build effective partnershipsduring the transition process.
